In the wake of the protests related to George Floyd’s death, there have been many calls to “defund police.” If we’re talking about institutions that are supposed to protect us but too often cause us harm, maybe we should be talking about defunding health care as well. In fact, the disparities driving the calls to defund police also underlie health outcomes; with COVID-19, these social determinants of health have contributed to the wide gap in African-American fatality rates. It’s time to make our healthcare system less about medical care and more about health.

A report this month published in the British Medical Journal found that 80% of 293 physician leaders and board members of 10 of the most influential medical associations in the United States received financial payments of $130 million in total for “leadership” activities between 2017 and 2019. As the major medical association leaders pad their pockets, our federal and state leaders are now forced to empty theirs. Our health care system has placed profit above patients at every turn, at the cost of rejecting universal healthcare.

With the exceptions of pediatrics and obstetrics/gynecology, women make up fewer than half of all medical specialists. The greatest challenge for young women in urology is to gain acceptance among older men who require examination of their genital region and often need surgery. For the barriers that women in the field still face, Dr. Krambeck breaks down three guideposts which have been set forth in paths paved by leading urologists.
